Dr. Epstein medical records

Dr. Epstein made some of his medical charts available to Advanced Menstrual Care Center and placed those charts into boxes.   A list of charts was not provided to AMCC.  As a result, AMCC cannot confirm possession of a chart until a search is initiated.  

Patients who wish to transfer their medical records, please note the following:

• As a courtesy to patients and their provider, AMCC will forward one copy of your records to one provider by fax without charge.  Requests for multiple copies, paper copies or personal copies will be charged consistent with Maryland law.
• All requests must be received in writing with your name, date of birth, current address, email and phone numbers.  
• The name of the doctor, address, phone number and fax number must also be included on the request.
• The request for medical records must be signed by the patient.  Electronic signatures and printed signatures are not accepted.
• Requests for medical records can be sent by fax to 410-321-5493, or mailed to: 1810J York Rd, #217  Timonium MD 21093.
